I've caught myself a couple of times with the grinder over the years (luckily through jeans or leather gloves so not too badly), but I have set myself alight a couple of times, and a mate landed himself at the local eye hospital by not wearing goggles.

Weld splatter can also do permanent damage, as can the various cutting tools available
